Support for Long-Term Conditions

A long‑term condition may be part of your health story, but it is not the whole picture. At QMC, we focus on early screening and prevention wherever possible, and support you if a long‑term condition develops. Care is shaped around you and your needs, which may include nurse appointments for annual checks and screening, and GP visits to review medications, symptoms, and overall wellbeing.

Support for Living Well with a Long Term Condition

Living with a long term condition can feel complex without the right support. Our team can help you stay on top of your health with regular monitoring, screening, and guidance that fits your needs. Over time, this may include nurse appointments for annual checks, GP visits to review medications and symptoms, and referrals to wider health services when needed.

Long Term Conditions We Support

Some examples of long term conditions we support patients living with include: diabetes, cardiovascular disease, severe and enduring mental health (this is classifications based on DSM 5 which is very specific, please check if you meet this criteria), respiratory disease such as asthma, and gout.

How our clinical team works together

Long-term condition care is delivered according to the needs of the patient. For example, the patient might have some time with a nurse to do annual checks and make sure things such as retinal screening and foot care have not been missed, and then also GP visits to address medications and symptoms.

Our team will work with the patient to design care that fits their long term condition and needs.

How external teams are involved

We may refer to the wider health care team for support such as home help assessments, dieticians, podiatry, community support, and other funded WellSouth programmes patients may be eligible for, such as help managing or living with type 2 diabetes or pre diabetes. HIPs and HICs are also part of the QMC wider team who can support patients needs.

Some funding is available through WellSouth if patients meet the criteria and depending on allocation of spaces/funds.

How to start getting support

Much of what we see already is a long term condition, so many patients will just come to their GP as normal. There is nothing special they need to do. If they already have an eligible long term condition they may be identified by a doctor or a nurse during routine care.

Patients are always welcome to ask if they think they meet criteria for funding, but access to funded appointments depends on meeting strict eligibility requirements and current capacity.

Managing your long term condition at QMC

Our nurses, GPs, and wider health team can help you stay on top of your long term condition with regular checks, medication reviews, and referrals when needed. Our team have systems to identify patients who meet the criteria and staff looking through those systems.

There is nothing extra you need to do to begin. Most patients simply book their usual GP appointment.

What is a long term condition?

A long term condition is a health issue that lasts for six months or more. Examples include diabetes, cardiovascular disease, severe and enduring mental health, respiratory conditions such as asthma, and gout.

Do I need to do anything special to get LTC support?

No. Most patients simply book their usual GP appointment. Your GP or nurse will guide you through the next steps.

How do I know if I am eligible for funded support?

Funding budgets and spaces are limited. Our team use internal systems to identify patients who meet the criteria. If you are eligible for funded programmes, we will let you know during your appointment. Funded is all allocated based on an equity lens.

Will I see a nurse or a GP?

It depends on your needs. You may see a nurse for annual checks and screening, and a GP for medication reviews or symptoms. Your care plan is built around you.

Can external teams be involved in my care?

Yes. We can refer you to dieticians, podiatry, community support, home help assessments, and other WellSouth programmes if the team feel these will suit your needs.

What should I bring to my appointment?
