Enrolled QMC patients can access free Wellbeing & Lifestyle Support through Tōku Oranga (Southern Access and Choice), offering friendly, practical help from our Health Improvement Practitioners and Health Coach for anyone managing stress, low mood, or long-term health conditions—no referral needed, just support focused on what matters most to you.

In-practice support that’s easy to access
Free, in-practice wellbeing and lifestyle support for enrolled QMC patients — no referral required.
Whether you’re dealing with stress, low mood, sleep issues, or lifestyle changes, the team is here to listen and guide you toward practical, meaningful steps forward. You don’t need a referral, and most patients can be seen within a few days.
Services available include:

Health Improvement Practitioners (HIPs):
Registered health professionals who provide practical, short sessions to help you learn tools and skills for managing stress, anxiety, sleep, mood, grief, and everyday challenges. It’s about finding small, realistic steps that work for you.

Health Coaches:
Work alongside you to build small, sustainable changes around exercise, nutrition, sleep, and managing long-term health conditions, like diabetes, heart disease, or chronic pain. They work alongside HIPs to offer a safe, supportive space and take small, meaningful steps toward feeling better.

Community Support Worker:
At QMC, we’re proud to offer access to a Community Support Worker—someone who can help with the practical things that make a big difference to your wellbeing.
They’re here to walk alongside you, offering support with non-clinical challenges like housing, employment, Work and Income, and connecting with local services. Whether you’re feeling stuck or just need a hand navigating what’s out there, they’re here to help.
How it works:
Referrals come through your GP or nurse, and appointments can take place at the medical centre, in your home, or out in the community—whatever works best for you.

When to Reach Out
You don’t need to wait for a crisis. Reach out if you’re experiencing:
- Stress, anxiety, or low mood
- Sleep problems
- Lifestyle challenges (e.g. quitting smoking, managing weight)
- Parenting or relationship difficulties
- Support needs related to long-term conditions (e.g. diabetes, heart disease, chronic pain)
The Health Improvement team are here to help you take the first step toward feeling better.

Call us on 03 441 0500. Our HIPs and Health Coaches do move around between our practices, so please specify where you would like to see the practitioner and we will do our best to accommodate you.
What to expect
Sessions are free for enrolled patients, typically 30 minutes long, and focused on practical strategies and short-term support.
